Practice Website Access Review — Service Terms

Last Updated: September 2026

By purchasing the Practice Website Access Review (the "Service") from Good Permutations / John C. Ollila ("Consultant"), you ("Client") agree to the following operational terms:

1. Scope of Service

The Service is strictly limited to an independent, written operational review of up to ten (10) publicly accessible web pages provided by the Client. The Service assesses public-facing friction and navigation. It does not include backend software configuration, technical implementation, SEO services, or website development.

2. Prohibition on Protected Health Information (PHI)

Client agrees not to submit Protected Health Information (PHI), account credentials, patient records, portal access, or other confidential clinical information. If Client nevertheless submits such information, Consultant may delete or return it without review, suspend the Service, and request a non-sensitive replacement submission.

3. Delivery Standard & Clarifications

Delivery of the PDF report is targeted within three (3) business days after Consultant receives both payment and a complete, non-sensitive intake submission. Business days exclude weekends and U.S. federal holidays.

The fee includes one written follow-up exchange by email, strictly limited to clarification of the delivered report. Additional analysis, meetings, implementation work, vendor evaluation, or ongoing consulting require a separate written scope and fee.

4. Fees, Cancellations, and Refunds

The flat fee is due at checkout. If the Service cannot be completed because the submitted website is unavailable, non-public, inaccessible, materially outside the stated scope, or accompanied by prohibited information, Consultant may request corrected non-sensitive materials. If the Service still cannot be completed, Consultant will cancel the engagement and issue a full refund. Otherwise, because this Service requires dedicated manual review time, the fee is non-refundable once the completed report is delivered.

5. Professional Disclosures & Limitation of Liability

Consultant provides operational and systems-design analysis. Consultant is not a licensed attorney, medical provider, or certified compliance auditor. No deliverables provided under this Service constitute legal advice, medical advice, technical cybersecurity testing, or a formal HIPAA compliance assessment. Client assumes all risk and responsibility for any operational changes made as a result of the Service.

In no event shall Consultant be liable for any direct, indirect, incidental, consequential, or special damages arising out of this Service. Client’s sole and exclusive remedy for any dispute shall be strictly limited to a refund of the professional fee actually paid by the Client.

6. Affiliate & Third-Party Referrals

The Service report may contain optional tool categories or specific vendor referrals to resolve identified friction points. Consultant may maintain affiliate or partner relationships with these vendors. If an affiliate link is provided, it will be conspicuously disclosed in the report. Client is never under any obligation to utilize recommended vendors. Consultant is not liable for the performance, security, or compliance of any third-party tool selected by the Client.

7. Governing Law & General Provisions

This agreement shall be governed by and construed in accordance with the laws of the State of California, with venue residing exclusively in El Dorado County, California. If any provision of these terms is held invalid, the remaining provisions shall remain in full force and effect. The payment confirmation, the product scope page, and these Service Terms constitute the entire agreement regarding this Service.
